Primary Care

Cedarcrest Health operates primary care practices in Harlow Falls, Renner, Maple Glen, and Bramford, making it easy for patients throughout the region to find a medical home close to where they live and work. Our family medicine and internal medicine physicians provide care for patients of all ages, from newborns through geriatric patients.

Each of our primary care offices is staffed by physicians, nurse practitioners, and physician assistants who work as a team to manage chronic conditions such as diabetes, hypertension, and asthma, while also providing preventive care including annual physicals, immunizations, and cancer screenings.

Our primary care network offers:

• Same-day sick visits at most locations
• Chronic disease management
• Annual wellness and Medicare visits
• Sports and school physicals
• Referrals to Cedarcrest specialty services

Patients can manage appointments, request prescription refills, and view lab results securely through the MyCedarcrest patient portal, which launched in 2010 and continues to expand its features each year. Our staff is happy to help new patients enroll in MyCedarcrest during their first office visit.
