Medical Records
Cedarcrest Health maintains the confidentiality of your health information in accordance with the Health Insurance Portability and Accountability Act (HIPAA) and Pennsylvania law. Patients have the right to request copies of their medical records, and our Health Information Management (HIM) department is available to assist with these requests at both Cedarcrest Medical Center and Renner Community Hospital.
To request your medical records, you will need to complete and sign a HIPAA Authorization for Release of Medical Information form. This form is available at any hospital registration desk, through the MyCedarcrest patient portal, or by contacting the HIM department directly. Requests made on behalf of another individual, such as a family member, generally require additional documentation, such as a power of attorney or guardianship paperwork.
Completed authorization forms may be submitted in any of the following ways:
- Through the secure document upload feature within MyCedarcrest
- By fax to the Health Information Management department at (814) 555-2277
- By mail to the Cedarcrest Medical Center HIM department
- In person at either hospital’s HIM office during business hours
Many patients find that the fastest way to access recent lab results, imaging reports, visit summaries, and discharge instructions is through MyCedarcrest, our free online patient portal. Records available through MyCedarcrest are typically accessible within a few business days of your visit, at no charge.
For records not available through the portal, such as older paper charts or records predating our current electronic health record system, please allow up to fifteen business days for processing, consistent with Pennsylvania regulations governing medical record requests. A reasonable copying fee may apply for large record requests, though fees are waived in certain circumstances, including when records are needed for a Social Security disability claim.
